ST Tammany Parish Animal Services is seeking one or more professional veterinarians to perform Veterinary services for the Parish’s Department of Animal Services. The STP Animal Services is a shelter that provides care, vaccinations, spay/neuter services while temporarily housing animals. Provider will provide routine veterinary services during normal business hours as needed on an average of three weekly visits at designated hours (approximately 24 hours weekly service). Services will include, but are not limited to the following: -provide vaccinations; -diagnostic testing, and treatment of illnesses for all animals; -provide veterinary care for injured and/or sick animals; -perform surgery limited to spay/neuter; -minor laceration repair for animals; -prescribe medication, as needed, for sick or injured animals; -perform pediatric spay/neuter surgeries; -establish a protocol for vaccination, -de-worming, and heartworm prevention; -determine whether severely injured animals need to be euthanized for humane reasons prior to the expiration of the holding period; -examine and provide treatment and make recommendations for large animals; and -perform exams on animals seized for cruelty within 24 hours of the initial seizure and present expert testimony in such cases, if needed, at trial. The Provider will be available for medical emergencies after normal business hours for sick or injured animals housed at the shelter. The Provider will report to the Director of Animal Services and perform work as requested. The basis of payment will be an hourly rate for services, payable monthly. 6 2.2 Period of Agreement The Parish and Provider agree that the initial term of the contract shall be January 1, 2021 through December 31, 2021 with an option to renew for two (2) additional one (1) year annual periods, beginning January 1, 2022. Renewal is at the Parish’s option and upon acceptance of the provider. The term of any contract resulting from this solicitation shall begin on the date of the Parish President’s signature or approval in writing by the Parish President or designee. The contract shall terminate pursuant to the terms and conditions of section 5.31 herein, the terms and conditions of the contract, operation of law, as agreed between the parties, or upon satisfactory completion of all services and obligations described in the contract. Prior to commencing each work task, the Provider will be required to secure a written Notice to Proceed Letter from the Parish and must execute an acknowledgment that it will comply with all requirements of the funding agency, if applicable. 2.3 Price Schedule Each Respondent should submit their Hourly Rate for Veterinarian services during normal business hours and after-hour medical emergencies in a separate sealed envelope. 2.4 Deliverables The deliverables listed in Scope of Work/Services are the minimum desired from the successful Respondent. Every Respondent should describe what deliverables will be provided per their proposal and how the proposed deliverables will be provided. 2.5 Location The location the work/delivery/service is to be performed, completed and managed is at St. Tammany Parish Government Department of Animal Services, 30178 Hwy 36 Lacombe, La. 70445. The veterinarian must submit other documents pertaining to his/her qualifications with the proposal. The other documents required as part of the proposal are as follows: • Copy of veterinary diploma. • Copy of current veterinary license. The RFQ requires the service of an experienced veterinarian with a minimum of two (2) years of experience working as an accredited veterinarian in private practice or a state-approved teaching facility.
